POSITION SUMMARY: The Patient Services Navigator Team Leader is responsible for leading the Patient Service Navigators in regards to patients who are in transition of care after being discharged from the hospital or receiving care in the emergency department. The overall goal is to reduce the risk of hospital readmissions and to avoid unnecessary emergency department visits by navigating the patient through the healthcare system. The Patient Services Navigator Team Leader reports to the Patient Navigation Center Supervisor or designee. SPECIFIC DUTIES: Lead the Patient Navigator Transitions of Care Team by providing guidance and training in regards to Patient Navigator duties.
