Serve as a Physician (Primary Care) located within the Bay Pines VA Healthcare System assigned to the Primary Care Service Line. This position is for assignment at the Port Charlotte Community-Based Outpatient Clinic, located in Port Charlotte, FL. The Primary Care Physician/Provider is responsible for the continuing management of assigned patients to include examination, medical care management, coordination of consultation, referral of patients, development and operation of quality assurance, utilization programs including peer reviews, community clinic physicians, and services and physicians in the Bay Pines Healthcare System. Duties and responsibilities include, but are not limited to: Providing primary care to a daily or extended assigned defined panel of primary care patients. Provide direct patient care full time and work as a member of an expanded healthcare team (social worker, dietician, clinical pharmacist, behavioral health clinician, and a nurse care manager) to oversee and coordinate medical care for a population of Veterans in a group practice setting utilizing the Patient Aligned Care Team model. Responsible to contribute to the achievement of quality of care standards, efficiency, process improvement and patient satisfaction within the Primary Care Division Coordinates preventive health care needs according to guidelines established by the Department of Veterans Affairs and sound medical practice. Measurement of clinical performance based upon quality reviews, completion of clinical reminders in CPRS, participation in medical center committees, and other methods. Knowledge of the patient care processes of the Department of Veterans Affairs, the Healthcare System and its clinics is maintained. Knowledge of the medical records system (CPRS). Knowledge of and abide by Healthcare System Bylaws, VHA Rules and Regulations and have a familiarization with existing VA specific clinical guidelines. Responsibility to accommodate assigned patient's emergent needs. Supervision of Physician Assistants (PA), ARNPs and Residents. Participate in interdisciplinary team meetings and functions as a team member. Management of the primary care profiles to insure appropriate timeliness and care including maintenance of health and treatment of illness. Responsible for delivery cost-effective care and referring patients to other specialized services only when appropriate. Work Schedule: Monday through Friday, 8:00 am to 4:30 pm.
