The Michael E. DeBakey VA Medical Center (MED VAMC) located in Houston, Texas is recruiting to fill a Physician for the Home Based Primary Care Program (HBPC). Internal Medicine, Primary Care, and Family Practice Physicians welcomed to apply; Fellowship training in Geriatrics is highly desired. The incumbent reports to the HBPC Medical Director. This position may be eligible for the Education Debt Reduction Program (EDRP). This opportunity is located within the Home Based Primary Care Program. The incumbent will be required to travel to their assigned patient panel home and provide all aspects healthcare for his/her patient panel, as well as clinical oversight of the assigned Nurse Practitioners/Physician Assistants who function within HBPC. Visits also include telephone visits and occasional tele face-to-face encounters. Duties of the position include, but are not limited to: Provide patient care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness, treatment of disease, and care at the end of life. Ensure practice-based learning and improvement, practitioners are expected to be able to use scientific evidence and methods to investigate, evaluate, and improve patient care. Demonstrate interpersonal and communication skills that enable them to establish and maintain professional relationships with patients, families, and other members of the health care team. In collaboration with the HBPC team, is responsible for assuming full primary medical responsibility for assigned HBPC patients, identifying medical problems, defining the medical management of these problems, determining the need for consultation from medical, surgical, and/or psychiatric subspecialty clinics as needed. Provides medical oversight including review and approval of documentation requiring physician signature for the HBPC patients. Meeting all performance measures pertinent to HBPC, including, but not limited to clinical reminders, wait times, average daily census, external peer review program. Incumbent is responsible for meeting home care accreditation standards, including, but not limited to VHA and the Joint Commission standards for home care, for ensuring that the primary care services provided are accessible, comprehensive, coordinated, longitudinal, accountable, and acceptable. Other duties as assigned by supervisors and based on employee competence and organizational needs. Malpractice: Physicians are covered under the Tort Claims Protection Act. Approved Incentives: A recruitment/relocation incentive may be authorized for a highly qualified candidate. Paid Time Off: 8 hours Annual Leave accrued per biweekly pay period; 4 hours Sick leave accrued per biweekly pay period; 10 paid Federal holidays.
