This fulltime position consists of a Psychiatrist to work within the Primary Care Mental Health Integration Team (PCMHI) to assist in the delivery of evidence- based care for Primary Care patients identified with a variety of behavioral and mental health issues including mild to moderate depression, anxiety and substance use misuse. Duties include, but are not limited to: Performs psychiatric evaluations and functional assessments consistent with an integrated care model for a diverse population, including geriatric and post deployment combat Veterans, who are mentally ill and have psycho-social issues such as homelessness, unemployment, and poor family support network. Provides a full range of therapeutic interventions including diagnostic assessment, medication evaluation and management, suicide risk assessments, crisis intervention, and overall care of patients with psychiatric disorders. Ensures practice of psychiatric medicine is consistent with TJC, CARF, HIPPA, VA policies and procedures, clinical practice guidelines, national statutes and regulations, medical record documentation, and mandatory continuing education. Provides a full range of psycho-diagnostic services including assessment and the most appropriate psychotherapeutic techniques in providing quality care. Coordinates consults and triage of primary care mental health integration consults. Participates effectively in team meetings and treatment planning conferences, and collaborates with multidisciplinary team members that enhances coordination of comprehensive patient care. Orders diagnostic tests and consultations, and discriminates between normal and abnormal findings and makes appropriate treatment decisions. May participate in supervision and teaching in support of the psychiatric residency program, performs on call duties, and performs other directly related duties as assigned. Areas which the psychiatrist is assigned and has clinical privileges to practice may include depression, anxiety, psychosis, PTSD, substance abuse, and other psychiatric disorders in various clinical settings Provides chart based consultation, including E-consults as needed/requested. Maintains availability for informal and "curbside" consults as needed by primary care staff. Ensures ongoing availability for urgent or emergent situations Routinely attends PACT huddles for informal consultation and education of the team Offers and provides formal education sessions as desired by other team members Provides ongoing training, supervision and backup for care management (CM). Provides consultation to co-located collaborative psychologists, social workers, nurses, clinical pharmacists and other disciplines as needed. Psychiatrists are expected to be able to assist with understanding the often complex interaction resulting from comorbid psychiatric and nonpsychiatric conditions, as well as identify psychiatric symptoms induced by other illnesses or medications. Assures that the medical workup ordered by the primary care provider is sufficient and adds to that workup if indicated.
