Accountable Health Partners (AHP) is seeking an experienced healthcare professional to join our CMO as the Senior Director of Care Management. Created in 2013, AHP offers a broad network of over 2000 providers associated with eight of our region’s leading hospitals and over 100 primary care practices. Accountable Health Partners is the accountable care network affiliated with UR Medicine and works across health system to deliver population health initiatives. AHP offers a collaborative team dedicated in its mission to improve quality, access and affordability of healthcare services.
The Senior Director of Care Management serves as AHP’s senior clinical leader overseeing care management activities across the AHP and UR Medicine network. In collaboration with medical, nursing, behavioral health and social work leaders throughout affiliated hospitals, primary care and specialty practices, the Senior Director of Care Management will develop strategic plans related to Population Health Management. The Senior Director of Care Management serves as a voice for innovation, overseeing clinical initiatives aimed at addressing social determinants of health.
Primary areas of focus include:
Developing and implementing approaches to care management that achieve high quality care, reduce variations in care delivery and meet financial goals related to value-based payment.
Directing a multi-disciplinary care management team that includes clinical providers across all divisions of UR Medicine and AHP.
Creating consistent clinical protocols across the care management structure, reducing unwarranted variation. Will share best practices across the clinical enterprise to meet targeted goals.
Fiscal responsibility for the care management program, assuring business plans and goals are achieved in a cost-efficient manner, and that care management resources are deployed efficiently and effectively.
Developing tools for feedback from providers and patients regarding the effectiveness of the care management program, and developing action plans to correct identified issues.
Fostering relationships with community organizations to see that both patient needs and clinical/financial outcomes are met.
Master’s degree in Nursing or Health-related field; doctoral degree preferred
10 years of relevant work experience including 3+ years’ management/supervisory experience in a health system or health plan setting
Prefer active RN license in the state of New York (or ability to obtain NY license)
Strong clinical knowledge of medical practice settings and healthcare delivery systems
Knowledge of utilization and care management principles and guidelines
Utilization management and behavioral health experience desirable
